To book an appointment, you will need a referral for a consultation to see Dr Gorai either from your General Practitioner or a Specialist Physician.

 

Referrals need to be faxed or emailed to the main rooms in Bundoora prior to making an appointment, once the referral has been received, the main rooms receptionist will contact you as soon as possible to make an appointment.

If your referral has been sent to the main rooms by your referring GP/Specialist, please feel free to call our friendly staff on 03) 8582 6945 and they will make an appointment for you at a convenient time.

 

Please feel free to download a copy of the New Patient Registration Form to fill out before your appointment. Be sure to bring it along with your referral.

Workers Compensation and TAC Patients

Please note: Workers Compensation and TAC patients will only be given an appointment once Dr Gorai's rooms have received written approval from the insurance company. The insurance company may fax, email or post the approval letter to:
